Scheduling an appointment is simple. Call or text the office (518)-871-9097 and the staff will connect you to our dedicated new patient onboarding specialists. They will be able to answer any questions you might have and match you with the therapist who can most support you and your needs. If you prefer, you may also email us using this form.

On the day of your first appointment, arrive about 10 minutes early, so you can get squared away with the front desk before you meet with your therapist. Most of our new patient forms will have been emailed to you ahead of time, so only a few quick signatures and your copay will need to be addressed at that time. If you aren’t comfortable with online forms, we can help you fill things out at the office.

Once you are settled at the front desk, your therapist will greet you and take you to your treatment room. Your evaluation will begin with a detailed review of your history - from birth to now! Your therapist will want to know what kinds of injuries you have had, even ones that might be decades old. These injuries are just as important as the more recent ones. What if you can’t remember every injury?? It’s ok! Just remember what your can. The injuries that you can recall the entire story are usually the ones the body is dealing with. Your therapist will help you decode the rest!

Once your evaluation is complete, your therapist will create a specialized treatment plan for you and explain the rationale for the treatment. Don’t be surprised if the place that hurts does not get the immediate attention! Many times, the part of the body that hurts is the part that is doing all the work. Your therapist will find the part(s) of your body that can’t work, which is causing the hurting part of your body to overwork. During the course of treatments, balance will be restored in the body and you will feel better.

Your therapist will suggest to you a treatment plan including frequency and duration of your visits. Generally, is it suggested to start with about 12 sessions. This will give both you and your therapist a good idea of how your body is handling the treatment. It is important for you to share your goals and level of progress with your therapist, so that any necessary adjustments can be made.

Our Shockwave Treatment can be broken down into the following categories:
Test Drive Visit: $49 (Trial Shockwave Visit)
Treatment Cost - 1 Joint
$150 per visit
$1350 for a 10 visit package* (10% off = $150 in savings)
Treatment Cost - 2 Joints
$240 per visit
$2160 for a 10 visit package* (30% off = $840 in savings)
*Shockwave pre-purchase membership: Package must be purchased in advance of the visit to take advantage of the savings. Bonus: After the completion of the plan, you will be able to purchase visits for $100/joint for up to 1 year from your “test drive” visit date.

CDPHP, MVP, Blue Cross ( Anthem, Empire, Mediblue, & Forever Blue), Cigna, Medicare, Humana, Worker’s Compensation and No Fault.
We can directly bill your insurance. Your co-pay and authorized visits will be determined by your plan with your insurance company.
Each insurance company has different policies for physical therapy. It is strongly recommended that you review your benefits information. Any questions regarding coverage should be directed to your insurance carrier directly. Please bring all insurance cards and therapy prescriptions with you to your first appointment.
Scripts will be required after 30 days, according to NYS Direct Access Laws.

Tricare, NYSHIP/Empire Plan, Aetna, United Health Care, Highmark NENY, Federal BCBS, UMR
Although we are OUT-OF-NETWORK with certain insurances, you are still able to submit for OUT-OF-NETWORK benefits.
"OUT-OF-NETWORK" means that you will pay our self-pay rate, but are able to submit the bills on your own to your insurance company for reimbursement.

Yes! You are able to still be seen as a patient. We would have you pay our self pay rate for treatment. Following each session, we will give you a super bill which contains all of the necessary documentation you will need to submit for reimbursement.

Our self-pay option is available to you if we do not participate with your insurance, you do not have insurance, or if you go over the number of authorized visits allowed by your insurance plan.
Listed below are the prices for our self pay sessions:
20 Minute Session: $65.00
40 Minute Session: $115.00
60 Minute Session: $165.00
A script will be required after 10 visits or 30 days from your first visit. Direct Access is available. Direct access means that for therapists that qualify, you can be seen for physical therapy without a script for 10 consecutive visits or 30 days, whichever comes first. After that time period, a script must be obtained, even if it is not required by your insurance company. This is NYS law.
