Core is different than any other physical therapy practice. We have a different scope in our evaluation and we have different treatments to address your problems.

The more details you can provide about your medical history the better. Old accidents, sicknesses and symptoms may be relevant so please share as much as you can.

WE CAN DIRECTLY BILL YOUR INSURANCE AND YOUR CO-PAY. AUTHORIZED VISITS WILL BE DETERMINED BY YOUR PLAN WITH YOUR INSURANCE COMPANY. EACH INSURANCE COMPANY HAS DIFFERENT POLICIES FOR PHYSICAL THERAPY.  IT IS STRONGLY RECOMMENDED THAT YOU REVIEW YOUR BENEFITS INFORMATION. ANY QUESTIONS REGARDING COVERAGE SHOULD BE DIRECTED TO YOUR INSURANCE CARRIER DIRECTLY.

PLEASE BRING ALL INSURANCE CARDS AND THERAPY PRESCRIPTIONS WITH YOU TO YOUR FIRST APPOINTMENT. SCRIPTS WILL BE REQUIRED AFTER 10 VISITS OR 30 DAYS ACCORDING TO NYS DIRECT ACCESS LAWS.
